Ameriplan® BENEFITS OPTIONS for EMPLOYERS                  
 
 
                                              
 
Employers can provide Ameriplan Health® and  Ameriplan Dental® benefits (including Vision,
Prescriptions 
and Chiropractic) for their employees under the following options:
 
1)    Payroll Deductions - This option is available if 5 or more employees signup with Ameriplan®. This
involves a one-page Employer Acceptance Letter, and an Ameriplan® List Bill for Employee
Groups
.  If, for example, 10 employees signed up for Ameriplan Dental® Household benefits, the $19.95/mo
 membership fee would be deducted from each employee's pay, and the Employer would send a check for
$199.50 to Ameriplan® along with a List Bill (prepared by Ameriplan®), listing the 10 employees.
 
About 20 days prior to the 1st of each month, Ameriplan® sends the Employer the List Bill showing each
employee included in the plan. Any employee additions are added to the List Bill, and employee terminations are
crossed out. The modified List Bill and a check for the proper amount are sent to Ameriplan® prior to the 1st
of the next benefits month.
2)    Employer Sponsorship - With this option, all the Employer does is inform the employees that
the Ameriplan Benefits option is available to all employees. The  employee can elect to signup with Ameriplan®
on their own, and pay the membership fees by electronic credit or  debit  card  or bank account draft.  
Ameriplan®  Brochures with  application  forms  would  be distributed to employees. 

3)    Employer Endorsement - With this option there would be no Employer involvement other than
to notify
employees by Email or Memo that Ameriplan® Benefits are a supplemental option. The message
would
identify the Ameriplan® Broker contact, and an Ameriplan® website for employees to reference, and
signup online for Ameriplan Dental® and Ameriplan Health®. This option works very well for companies with
multiple locations.
 
4)    Cost-sharing - For those Employers who are willing to contribute toward the cost of Ameriplan®
membership,
this option would operate in the same manner as 1) above. Using the 10 employee example
in 1) above, the Employer pays a portion of the $19.95 membership fee on behalf of each employee. The 
Employer would
still send a $199.50 check with matching List Bill to Ameriplan®, and the employee payroll
deduction would be reduced by the amount of the Employer contribution.
 
 
With all of the above options, the Ameriplan Dental® Benefit can be cancelled at anytime. There is no annually
renewable contract as is commonly required with dental insurance. Plus, the Ameriplan® membership fee
is guaranteed for two years, and in fact, has not changed during the past 7 years.
 
Finally, there is a one-time $20 processing fee for all individual Ameriplan Dental® signups and a one-time
$30 processing fee for Ameriplan Health® signups.  However, except during special promotions, Ameriplan® 
can waive this fee for groups of 5 or more employees signing up with Ameriplan®.  Thus the Employer
can provide
a $20  or $30 savings for each employee simply by allowing them to pay the membership fees through
payroll
deduction as in 1) above. Also, since Ameriplan® requires electronic payment of membership fees for individuals
and families, this provides a means for employees without credit cards or bank accounts to get
benefits by way
of payroll deduction.
